Approximately 21.4 percent of Americans lack access to home internet. Of the more than 24 million unconnected households nationwide, this digital divide disproportionately affects minority and low-income populations.

Our overarching goal is to remove barriers to education and employment and build the digital capacity of those in the community most acutely affected by the digital divide.

In partnership with the City of Sacramento, SMUD, and other community-based organizations and non-profits, United Way is helping to bring internet services, computers, and digital training at no cost to eligible residents. Filling out this form will assist United Way in determining applicants' eligibility to receive digital resources.

To be deemed eligible to receive resources, you must fit the following criteria:
  • Must be eligible/ on public assistance
  • OR have an income of $30,000 or less
  • Live within the City of Sacramento
Additional requirements may apply.

For each of the following questions, please choose the best answer or response (by checking the box or boxes) for your experience or opinion. Even if you rarely or never use technology or the internet, please follow the instructions and answer all the sections of this survey.

If you need assistance filling out the survey, please call 211.
